• Include the page URL, the fact you believe is wrong, and a source we can check.

What happens next

• A researcher re-checks the claim against primary sources.
• Confirmed errors are corrected and the page's last-reviewed date is updated.
• Material corrections to published articles are noted in the article's source notes.
• Providers may request corrections to their own profiles through the claim flow; supporting documentation is required.
